All of our registration forms are available below to download. These can be filled in on your phone or computer and emailed to us. Alternatively, you can print them and then complete, then post them back to us.
Please complete each form otherwise we will be unable to register you.
Each person will need to complete their own set of forms and we have separated the adults forms from the children's ones below.
To email them back to us, please send them to manorpark@nhs.net with “New patient –Registration forms” as the subject. For families or households who are registering, please send in a separate email for each person along with their documents.
You can fill in forms on iPhones and iPads using Markup, see https://support.apple.com/en-gb/HT206885
You can fill in forms on Android phones using a variety of apps such as Adobe Fill & Sign or Google docs
You can open the forms on your desktop computer and use the Tools menu in Adobe reader to complete the forms.

1: New Patient Medical History Form
2: GMS1 family doctor services registration form
3:Communications Consent Form, text and email
4: Online Services (Adult) - prescriptions, appointments etc
5: Summary Care Record opt in or out

Children (0-15 years):
Please print and complete all of these following 4 or 5 documents (number 3 only if appropriate):

5: Summary Care opt in or out
